Collect missing attendee details with a certificate request form

Share Verifycate's Issue Request Form after an event. Applicants confirm their email and submit details; you choose whether requests are issued automatically or reviewed first.

The event is finished, but a few attendee names never made it into the spreadsheet. Instead of collecting names across chats and email, share one Issue Request Form link. The glossary calls this a Certificate Request Form; in the product, the setup label is Issue Request Form and submissions appear in Issue Requests. Applicants confirm their email with a one-time code before submitting.

In the glossary: Certificate Request Form.

How it works

Issue Request Form

  1. 01

    Set the form fields

Choose the information to collect, such as name, email, institution, team, or event details. Answers can fill the certificate's matching Attributes.

  2. 02

    Share the Issue Request Form

Send the public link through your event channels. Applicants confirm their email with a one-time code, then submit their details without creating an account.

  3. 03

    Choose how to issue

Select Issue Automatically or Review Before Issuing. Review submissions in the Issue Requests inbox; approving a request issues and emails the certificate.

Common questions

Can I review a request before issuing?
Yes. Choose Review Before Issuing. Submissions appear in Issue Requests, where you can accept and issue, reject, or reopen a request.
Can requests issue certificates automatically?
Yes. Choose Issue Automatically. Anyone who submits the form receives the certificate right away.
Can I collect extra event details?
Yes. Configure additional fields for details such as institution, team, role, or session.
Is an Issue Request Form the same as an Unlock Form?
No. The Issue Request Form is the product label for the public Certificate Request Form. It lets applicants ask for a certificate. An Unlock Form goes to recipients you have already added and collects a response before their certificate is released.
